Credit card authorization form
Clients can by credit card even when you're away from your credit card machine or computer - great for in-home treatments and on-site consultations. For pay-in-advance services, or to pre-authorize a lump-sum service, email the form and have clients to fax this back for your records and to hold their appointment.

New patient registration form
For healthcare and complementary medicine practices. Includes comprehensive medical history forms. Note: This form is in html format to upload onto webserver or email to patients. Tip: Hosting the page on your website is the better option since you won't know if your patients have html-based email or not.
